GUARANTEE TERMS & CONDITIONS
30 DAY LENS GUARANTEE
- If not satisfied with your lenses we will remake them.
- If the new replacement lens price is higher – Patient only pays the difference.
- If the new replacement lens is less – We will refund the difference.
30 DAY FRAMES GUARANTEE
- If not satisfied with your frame you can pick out a new frame.
- Frame Re-Style – 30 day restyle into any new frame.
- If the new replacement frame is higher – Patient only pays the difference.
- If the new replacement frame is less – We will refund the difference.
- Frame and/or lenses must be in the same condition as when purchased. Any defects other than manufacture defects voids the guarantee.
DOCTOR RE-EVALUATION RX CHECK
0-30 Days: INCLUDED – NO CHARGE.
- 1 month-6 months: $45 OV fee due to evaluation of eye health and prescription.
- 6+ months: Complete Exam Required due to changes in eye health and prescription.
LYMER VISION EYEWEAR PROTECTION PLAN
Instructions: You must keep this Protection Plan and sales receipt to present at the time you are utilizing the plan. This Plan, including the terms, conditions, limitations, exceptions and exclusions, the sales receipt, commencement date, and product identification, constitute the entire Plan agreement.
Term of Coverage: The term of your Plan commences upon the date of purchase of the product and continues for one year.
What is Covered: This Plan covers unlimited repairs for 12 months, cracked &/ or coating peel on lenses, cracked frames, breakdowns due to normal wear and tear, and defects in materials and workmanship. If we determine your glasses cannot be repaired, we may replace your frame &/or lenses with a similar frame &/or lenses of like kind and quality that performs to the factory specifications of the original glasses, or at our discretion, we may issue you a check for the original purchase price of your glasses, excluding sales tax, as indicated on your sales receipt. Non-original parts may be used for repair of your glasses if the manufacturer’s parts are unavailable. There will be a co-pay, see the “Co-Pay” section below for more information. At our sole discretion, we may require that you return your glasses to us as a condition of receiving a replacement pair of glasses.
Co-Pay: If your product requires the following services, the retailer will collect the applicable co-pay amount from you based on the service you receive:
New frames =$25 New lenses = $25 New frames & new lenses = $50.
Transfer: This Plan is non-transferable.
Renewal: This Plan is non-renewable.
WHAT’S NOT COVERED
Coverage does not cover patient-owned frames of lens-only purchases. Warranty will cover lenses only for patient-owned frames.
